Mandated Dental Examination Program update

Dear Parent/Guardians/Caregivers,
We hope this email finds you well and that your child(ren) had a great start to the school year.
As informed in a previous email, beginning with the 2016-2017 school year, SCASD will be providing
a MANDATED DENTAL EXAMINATION PROGRAM. The Mandated Dental Examination Program
represents dental health services which students in grades kindergarten, third and seventh must
receive.
At this time parents/guardians have three options to proceed:
1. Parents/guardians can have their family dentist perform the examinations and report the results of the
examination on the Private Dentist Report form (see attached) and return to the school to be included in the
comprehensive school health record by December 16, 2016.
2. Parents/guardians can fill out the Notice of Dental Examination (see attached) and return to the school prior to December 16, 2016 informing the school of the intent to have the family dentist perform the examination. Following the examination, the Private Dentist Report form (see attached) will need to be submitted to the school to be included in the comprehensive school health record.
3. If unable to have a family dentist complete the form, parents/guardians can opt to have a dental examinations conducted at school by the school dentist, Dr. Killareski. The school examination complies with the School Health Law. There is no charge for this examination and parents may be present.
Please note: Examinations done by the family dentist within four months (up to one year with a Department of Health approved modification) prior to the opening of school are accepted for the required examination that year.
